Oculoplastic surgery

The oculoplastic surgery includes procedures involving the eyelids and the area around the eyes. It has two objectives: functional , to improve vision and protect the eye, and aesthetic , to restore a more rested appearance without weighing down or freezing facial expression. At Les Oliviers Eye Clinic in Sousse, Tunisia, we perform two major procedures in particular: ptosis surgery for a drooping eyelid and blepharoplasty for excess skin or eyelid bags.

Important to know: The aim is not to “pull” the eyelid, but to restore a natural balance while protecting the ocular surface.
Purpose Function + harmony of the eyes

Key point Natural result + follow-up

When should oculoplastic surgery be considered?

An eyelid that is too heavy or too low does not only affect appearance. It may reduce the visual field, cause forehead fatigue, and impair comfort. Conversely, excess skin or pronounced bags may create a permanently tired appearance even when you feel well. The indication is determined after a complete examination of eyelid position, symmetry, skin quality, dryness, and muscle tone.

  • Reduced visual field: Heavy eyelids or a “curtain” over the upper field of vision.
  • Functional discomfort: Eyebrow fatigue or the need to raise the forehead in order to see better.
  • Asymmetry of the eyes: One eyelid is lower or heavier than the other.
  • Desire for a more rested appearance: Excess skin, bags, or pronounced folds.
In oculoplastic surgery, operating well primarily means protecting the eye, respecting anatomy, and aiming for a natural result rather than transforming the appearance.

Our two techniques: ptosis surgery and blepharoplasty

The same operation is not used for every problem. Ptosis is a problem of eyelid height related to the muscle that raises it. Blepharoplasty treats excess skin and/or eyelid bags. Sometimes both problems are present, and the examination helps determine the most appropriate strategy.

Ptosis: a drooping eyelid

Ptosis may be mild or pronounced. It can interfere with vision, especially at the end of the day. Surgery aims to reposition the eyelid by acting on the lifting mechanism in order to restore a more natural height and symmetry.

  • Aim: Raise the eyelid precisely.
  • Benefit: A more open appearance and improved comfort.
  • Important: Balance with the other eye + protection of the ocular surface.

Blepharoplasty: excess skin / bags

Blepharoplasty removes excess skin and, when indicated, treats eyelid bags. The procedure is designed to lighten the eyelid without changing the character of the eyes while preserving good eyelid closure.

  • Aim: Lighten the eyelid and smooth the expression.
  • Result: A more open and rested appearance.
  • Important: A discreet scar in the natural fold.

Can ptosis surgery and blepharoplasty be combined?

Yes. This is sometimes the best option when the eyelid is both low because of ptosis and heavy because of excess skin. The plan is personalized to correct function without overcorrecting appearance.

How the operation is performed

Oculoplastic surgery is performed in the operating room under suitable anesthesia, often enhanced local anesthesia and sometimes general anesthesia depending on the circumstances. Incisions are placed in natural folds to make scars as discreet as possible. The surgeon checks symmetry and balanced eyelid closure.

  • Anesthesia: Comfort and safety during the operation.
  • Discreet incisions: In the eyelid fold or other natural areas.
  • Targeted correction: Muscle for ptosis and/or skin and bags for blepharoplasty.
  • Final check: Symmetry, closure, and protection of the eye.

Postoperative recovery: what is normal

Swelling and bruising are common after the operation and gradually decrease. A pulling sensation may occur. Eye drops or ointment and cold compresses, when prescribed, help improve comfort. The result continues to refine over several weeks as the tissues settle.

  • Swelling / bruising: Expected at first, with gradual improvement.
  • Local care: Hygiene and ointment or eye drops according to the prescription.
  • An evolving result: The appearance becomes more harmonious over several weeks.
  • Seek medical advice if you experience: significant pain, loss of vision, bleeding, or persistent intense redness.

Frequently asked questions

Will the scar be visible?

It is generally placed in the natural eyelid fold and fades over time. Healing quality also depends on the skin and postoperative care.

Is this purely cosmetic surgery?

Not always. Ptosis and some blepharoplasty procedures have a genuine functional effect on the visual field, fatigue, comfort, and eye protection. The aesthetic improvement is often an additional benefit.

When can I return to work?

This depends on your activity and how visible the bruising is. Many patients return gradually after a few days, according to comfort and medical instructions.
